Display name

1. Display name
Enter the Display Name.
 
The Display Name is a Required field ().  The user needs to enter a Description for the Phase. Should the user not enter a Display Name and attempts to Save () the information the system will give an indication  () that this is compulsory information.

Table Mapping

2. Table Mapping
Enter the Table Mapping - if Applicable.
 
The Table Mapping Allows the Category, linked Questions and Answers to be exported to a Data Table with the same table name as the Table Mapping.

Save/Cancel

3. Save/Cancel
When all information has been entered, the Category is saved by Left Clicking on the Save Button (). Should the user want to Cancel the Add Action, Left Click on the Cancel Button ().
